Are you interested in taking part in a new programme available within Bernaghmore Medical Practice?

 

Are you aged 65 years and over?

 

Do you live alone or with another older person in your own home or in sheltered housing?

 

Would you like to know more about support and services available within your local community?

 

Have you been diagnosed with two or more of the following conditions?

  • Diabetes
  • Respiratory 
  • Heart Failure
  • Coronary Heart Disease
  • Stroke
  • Hypertension
  • Parkinson’s
  • Osteoporosis

 

If these apply to you, you may be eligible for a new service we provide at the practice called IMPACTAgewell.  It is a programme which is designed to support you to improve your health and wellbeing.  A dedicated IMPACTAgewell Project Officer will talk with you about your general health and wellbeing, and support you to look at taking small steps that would empower you to age well now, and in the future.  Examples of  support  may include things like: health condition support groups, community transport, befriending visits, handyperson services, home security checks etc  If you are interested please contact the Practice staff on: 028 2531 3030 for further information.

Staff training

The practice will be closed on Wednesday 15th November 2017 at 1.00pm for staff training.

If there is a medical emergency please telephone the usual number on 028 25313030 and a message will be played back to you.  Either Dalriada Urgent Care will be covering or it may be a GP so please listen carefully to the phone message.

Down time

On Wednesday 29th March, the practice will have no computer access all day due to essential maintenance of our system. We would ask for your co-operation at this time and would appreciate if any repeat prescriptions could be ordered in advance if required. Only urgent requests will de dealt with. Thank you for your patience and understanding.

Christmas and New Year Holidays

The practice will be closed on Monday 26th and Tuesday 27th December for the Christmas holidays.  Dalriada Urgent Care will cover these dates but do not hold a stock of medication for repeat items.  Please ensure you have ordered your repeat prescriptions to tide you over the holiday dates.

We will also be closed on Monday 2nd January 2017 for New Year.   The same rules apply. Dalriada Urgent Care number is 25663500.

Flu Campaign

Please be aware that the practice is currently doing the flu vaccines for those patients over the age of 65 years and for those under 65 years who may suffer from chronic conditions – COPD, asthma, heart disease etc.  You can make an appointment with our Treatment Room Nurse or Practice Nurse or if you have an appointment with the GP they can also give you the flu vaccine.
